Define case management and care management and compare the differences.

Case management and care management are two terms which often appear together in the medical field, however they have distinctly different meanings. Case management is defined as “a collaborative process that assesses, plans, implements, coordinates, monitors and evaluates options and services to meet an individual’s health needs through communication and available resources to promote quality cost-effective outcomes” (Kulbok et al., 2011). Care management on the other hand is more involved with patient centeredness and self-care within a healthcare system. It focuses on the need of the client by providing personalized interventions with attention “directed toward promoting autonomy, development of skills for managing health conditions, increased access to needed resources including environmental supports and social support networks” (Kulbok et al., 2011).
